About Us:

Value-Based Wound Care
Built for What Comes Next

Old Mission Wound Care Logo

Old Mission Wound Care is a value-based wound care company built to deliver predictable costs and superior outcomes during a period of tighter reimbursement and increased regulatory oversight.

We partner with home health agencies, skilled nursing facilities, and risk-bearing organizations to deliver predictable wound care costs, measurable clinical outcomes, and reduced financial exposure.

Our model aligns incentives around healing, not utilization, so clinical decisions support both patient outcomes and financial stability.

Our Mission

To protect patients from avoidable complications, protect providers from unnecessary financial risk, and help healthcare organizations deliver better wound outcomes in a value-based world.

Why Old Mission
Wound Care Exists

For years, advanced wound care operated under a volume-driven, fee-for-service model. Previously operating as a leading mobile advanced wound care provider, we built a national mobile advanced wound care organization and delivered strong clinical outcomes, including a 98 percent wound improvement rate and a 1.3 percent wound-related rehospitalization rate.

However, even when delivered responsibly, that model rewarded utilization, created financial volatility, and increased exposure to regulatory and reimbursement risk.

In 2025, CMS tightened reimbursement and utilization controls around advanced biologics, leaving many organizations without a sustainable approach to wound care for their highest-risk patients.

Old Mission Wound Care was deliberately built to solve that problem.

Why Old Mission Wound Care Exists

Why We Built a Value-Based Model

We did not pivot because we had to. We pivoted because the old model no longer served patients or providers.

  • Predictable Costs: Create predictable, flat monthly costs per wound patient
  • Outcomes First: Align clinical decision-making with outcomes, not procedures
  • Reduce Risk: Reduce rehospitalizations, complications, and downstream expense
  • Executive Control: Give executive teams clarity, control, and confidence

This is wound care built for organizations managing risk, not chasing codes.

Treating High-Risk Conditions

  • Diabetic foot ulcers
  • Venous and arterial ulcers
  • Pressure injuries
  • Surgical and non-healing wounds

Our Clinical Foundation

Before launching our value-based model, Old Mission Wound Care operated nationally as a mobile advanced wound care provider, formerly known as Old Mission Wound Care.

During that time, we treated a high volume of chronic and complex wounds. This experience matters, because value-based care only works if the clinical foundation is real.

The Partners We Work Best With

We partner with organizations that are accountable for outcomes and cost, operate under PDGM, bundled payments, or capitated risk, and want fewer surprises and better visibility into wound care performance.

We are not built for high-volume, transactional wound care. We are built for alignment.
